Form W-4V was introduced in October 2014. It provides a way for employees to voluntarily withhold federal income taxes at lower rates than those specified for the employee by the Internal Revenue Service on Form W-2 (or other comparable form). Form W-4V requires that the employee's withholding rate be no more than a specified percentage (specified in the form or the statute) on the employee's total wages, which includes the net earnings of the employee (determined by subtracting any amount allowed under the Federal Insurance Contributions Act (FICA) from the employee's wages, from other income and, if required, other withholding). The percentage of the employee's wages to be withheld is determined by using either the employee's regular withholding rate (a separate amount) or the minimum rate specified under the employee's salary schedule for that year unless the employee is a covered employee who is exempt for federal income tax purposes.
